I have found what appears to be a hole at the top of my butt crack. What could this be?

It is hard to say what the hole on the top of the anal crack might be, without seeing it. The dimension of the hole as well as its depth would have been useful.

The particular location you mentioned does not contain any holes in the skin. Therefore, it might be a skin defect due to a previous pathological condition. You might have experienced local inflammation of certain of the skin structures that caused pus to collect in the tissue. Once the pus was drained, a hole was left in its place. The skin should have healed itself, but the stretching forces in the area did not let it happen.

You might want to visit a dermatologist to check for the hole. He might refer you to a proctologist if he suspects that the hole is actually a fistula (pathological canal) between the surface of the skin and the final part of the intestines close to the anus. If a fistula is diagnosed, a surgery might be needed to treat it. Otherwise, the hole might be insignificant enough for further treatment to be needed.
