Q: Poor circulation in the toes
asked by: DoctorQuestion on May 12th, 2009
Dear doctor,
6 day ago underwent an operation to remove a large amount of uric acid from one gouty toe in portuugal and since then told to keep the foot elevated but during my dressing change the nurse said i only have 80% circulation in the toe.
i would like your expert opinion, is it correct to keep the foot elevated if there is a circulation problem to the toe?
also your expert opinion with only 80% circulation to the toe, do you forsee amputation needed.

The circulation in the toe, in your case (existing gout), might be affected for several reasons. The removal of uric acid crystals from the toe might have caused a little damage to the tissue, which responded with inflammation and swelling. The swelling itself might have decreased the circulation. Second, if there were bandages put around the toe and were too tight, then they might have caused the bad circulation.

A doctor needs to see the toe for complete evaluation of the present condition and to detect the exact cause for the decrease in the toe circulation. He might decide where elevation is needed to decrease the swelling or it would be more beneficial not to elevate the foot any more in order to preserve the circulation that is left.
