Q: periumbilical pain

Is there any reason for having periumbilical pain and is shifted to right lower quadrant as the inflammation increases during the starting stage of appendicitis in a patient. What i mean is, why exactly patient feels pain at periumblical region during starting stage and after 5 to 6 hrs pain shifts to RLQ

Yes, specific somatic inflammation caused by localized irritation associated with more advanced appendicitis (ruptured or nearly-so) exhibits McBurney's point tenderness following sufficient luminal distension, impaired circulation, and possible intramural bacterial leakage resulting serositis. Until these steps occur, it's just the vague periumbilical pain resulting from referred visceral sensation (MacKenzie's theory).
