Q: Pelvic Inflammatory Disease or Ruptured Cyst?

My pain began on July 14th, 2008 which was a Monday. The pain was in my right abdomen just above my belly button. At first the pain was more of an annoyance than anything. I also felt nauseated and weak. Nevertheless, I made an appointment with my family doctor for Wednesday. The doctor initially thought that I had a bladder infection or UTI, although at this time he did no tests, and advised me to take AZO pills. By the next day, the pain was more intense and was sharp and constant, and migrated down to my lower right pelvic region. I went back to the doctor Thursday and he performed blood work, urine test, and a pelvic exam. My abdomen and pelvic area was very sore so he assumed that I had pelvic inflammatory disease and gave me a shot and prescribed me two different antibiotics for 14 days. All the tests that they performed showed up to be normal. My blood work was fine, no bacteria present in urine, and negative for gonnorhea and chlaymidea. I went into the ER early Saturday morning because my legs were shaking very bad and I couldn't control them. The ER doctor (who was a quack job) did all the same tests again that I had done in the doctors office--ughh and again they came back normal. The ER doc said I needed to give the antibiotics more time to work and sent me home with a prescription for a muscle relaxer and pain killer, she said the shaking was from the pain. The leg shaking continued during the middle of the night for three nights. On Monday I went back to the doctors office and he orderd a CT scan, to completely rule out appendicitis. The CT scan showed some free fluid in my pelvic region. I wasn't convinced my family doctor was giving an accurate diagnosis. To get a second opinion I went to a gyno/obgyn clinic on August 12, 2008. My doctor was supposed to send my files there, but apparently he never did. I saw a nurse practioner who said that because of my symptoms, all the tests being negative, and the fact that antibiotics didn't make me feel better after 6 days of treatment, she thought it was probably a cyst that had ruptured. A ultrasound tech did a transvaginal ultrasound and saw no more fluid, no signs of a new cyst, and said everything looked normal. The nurse said a lot of hometown family doctors automatically assume PID because they don't know what else it is. I explained that I am 18 years old and have been with the same male for three years and have never had unprotected sexual intercourse. My last annual pap smear was September of 2007 and it was normal. The nurse practioner prescribed me birth control because to prevent any more possible cysts. I am just confused and would greatly appreciate any insight on what any of you might think caused this pain. Was it PID, a cyst, etc? Let me know if any of you have had any of these similar symptoms. My family doc says PID but the nurse from the specialist office says it was probably a cyst? Thank you for taking the time to read this and for any replies.

I have the same thing!

For me, the pain started a few hours after I had sex one day. I had had surgery before for a cyst, so I was already on birth control to prevent anymore. I normally have horribly painful periods, and knew this feeling wasn't cramps. It was a very sharp painful stab in one particular spot - just like you described. I went to a clinic doctor who did exactly what yours did - tried me for STIs, urinanalysis, put me on antibiotics. Nothing helped, all the results came back negative. I went to my obgyn who did an ultrasound and found NOTHING. I talked to my dermatologist about this because I have lupus, an autoimmune disease, and thought maybe it had something to do with it, but he said probably not. He did recommend that if antibiotics don't work, stop taking them. There is no reason to take something that doesn't seem like it's working, espeically an antibiotic (if they can't even find a bacteria that's causing it), and you also don't want to build up tolerance to a drug.

It is about five months later and the 'PID pain' comes and goes. I'd recommend naproxen, see if you can get a prescription for a higher dosage than OTC (Alieve). It doesn't work for me, however my mom swears by it (she takes it for cramps and migraines).

I would love to hear from anyone else, I am as stumped as you! Eighteen years old, been with this guy a year and a half and we BOTH tested negative for ALL the STD/STI tests that we've gone through MULITPLE times... =/

Hi, I have had similar issues. Mine started w/ an abnormal pap years ago with a negative biopsy. Five years later with negative paps, I had a bad pap again and again with a negative biopsy. Four more years later I had a bad pap and refused a biopsy so after much fighting with my doctor who refused the CA125, I went for a sonogram which showed nothing. Now, five years later I have severe abdomial pain in my right lower side and had an ultrasound which found a 4.6x4.6 cm cyst on my left ovary. My doctor insists that it will go away on its own!? I haven't had my period in 36 days and have negative pregnancy tests via urine. I had blood work to rule out an ectopic pregnancy. What do you think will happen at my next pap...hmm. Whats with doctors!!!

Concerned about PID and Pregnancy at later age
Hi, first I’d like to say that I’m pleased that this forum is here, because I have felt so alone on this issue! I hope my issue/story does not bore you, but any advice is greatly appreciated:

I have not yet been tested for PID, but I am very concerned that this may be what I have.

Briefly, in my earlier years, I had one abnormal PAP and HPV-had biopsy and cryo—were successful. Years later HPV, but later exams showed negative.

Here’s my issue: I’m actually 43 years old; I look a lot younger (like 30), and people are usually shocked when I tell them I’m 43. I’ve been thru 2 divorces, and now married to an awesome guy (37).

During my single years, I dated other guys and was pretty stupid in having unprotected sex. STUPID! And in retrospect, I was so NAÏVE! I was a good teenager, got married the first time right at 20, and it wasn’t until we separated (11 yrs later) that I started to have unprotected sex. Again, I don’t know what I was thinking and how I could be so stupid.

Last year my GYN told me I tested positive for Chlamydia, so started treatment—we also had my boyfriend (now husband) on treatment. We moved afterwards, so I had my new GYN retest me: negative for Chlamydia, but positive for Gonorrhea! I couldn’t believe it! So my (now husband) and I took medication; later I tested negative.

I have no idea how long I had these before I finally got treatment--could be months or year??!!

Well, here I am now married to this awesome guy, and we’re serious about starting a family. I’ve had a feeling that it’s not going to be easy for me to get pregnant since it hasn’t happened yet, so we’ve been seeing a fertility specialist:

My progesterone level was fine and my cycles are regular-28. I started 5-day dose of Clomiphene (helps stimulate ovulation). One day last week, and just yesterday I was having piercing pains in my right lower abdomen which subsided some when I would sit down, but standing up or walking around, the pains were pretty piercing. Last week I had the pains on/off for 5-6 hours; yesterday I noticed the pain radiated toward the middle of my stomach, and it felt more like a Charlie horse cramp. Or could it have been something I ate???

So when I saw the doctor again, I told him that I had been reading a lot on the Internet and was concerned I might have PID (?) He asked why I did not tell him that I had previously been diagnosed with the 2 STDS, and truthfully, I suppose I was still in denial, or I thought he wanted to know if I “currently” had anything. Who knows….

But as I read more about PID, I’ve had the symptoms, i.e. bleeding between periods, abdom. pain, heavy periods, but no nausea or fatigue.

From my readings, those 2 STDS can cause serious damage to the reproductive organs, and I’m so afraid it has in my case. I don’t see the point in paying out of our own pockets for fertility tests when my tubes may be blocked or I have this PID, in which case if I do get pregnant, which I don’t know if I can since he said that my FSH level was really high due to my age—which means chances of getting pregnant are slim…
But I don’t know if I can go through an ectopic pregnancy, which could be lethal.
I’m in perfect physical shape – work out every day – and although I may feel like I’m 30, my eggs are 43 years old.

I think what I need to do is to make an appt w/my PCP about getting checked for PID. I’m very scared and pray I don’t have it, but I need to be realistic.

Earlier the specialist told me that we could schedule an X-Ray of my tubes (but not until June since he’s out all of May—(wait…wait…wait…frustrating), and could see if my tubes are blocked/damaged. Apparently this procedure is painful.

In reading some of your messages, I realize that your PID is somewhat a “medical mystery,” as you’ve not had unprotected sex, although I’ve read that one could still get PID from a bacterial infection (???)

I just wish I had been more wise in my earlier years.

Any insight or advice from you is greatly appreciated!
