Q: Pelvic Inflammatory Disease or Ruptured Cyst?

My pain began on July 14th, 2008 which was a Monday. The pain was in my right abdomen just above my belly button. At first the pain was more of an annoyance than anything. I also felt nauseated and weak. Nevertheless, I made an appointment with my family doctor for Wednesday. The doctor initially thought that I had a bladder infection or UTI, although at this time he did no tests, and advised me to take AZO pills. By the next day, the pain was more intense and was sharp and constant, and migrated down to my lower right pelvic region. I went back to the doctor Thursday and he performed blood work, urine test, and a pelvic exam. My abdomen and pelvic area was very sore so he assumed that I had pelvic inflammatory disease and gave me a shot and prescribed me two different antibiotics for 14 days. All the tests that they performed showed up to be normal. My blood work was fine, no bacteria present in urine, and negative for gonnorhea and chlaymidea. I went into the ER early Saturday morning because my legs were shaking very bad and I couldn't control them. The ER doctor (who was a quack job) did all the same tests again that I had done in the doctors office--ughh and again they came back normal. The ER doc said I needed to give the antibiotics more time to work and sent me home with a prescription for a muscle relaxer and pain killer, she said the shaking was from the pain. The leg shaking continued during the middle of the night for three nights. On Monday I went back to the doctors office and he orderd a CT scan, to completely rule out appendicitis. The CT scan showed some free fluid in my pelvic region. I wasn't convinced my family doctor was giving an accurate diagnosis. To get a second opinion I went to a gyno/obgyn clinic on August 12, 2008. My doctor was supposed to send my files there, but apparently he never did. I saw a nurse practioner who said that because of my symptoms, all the tests being negative, and the fact that antibiotics didn't make me feel better after 6 days of treatment, she thought it was probably a cyst that had ruptured. A ultrasound tech did a transvaginal ultrasound and saw no more fluid, no signs of a new cyst, and said everything looked normal. The nurse said a lot of hometown family doctors automatically assume PID because they don't know what else it is. I explained that I am 18 years old and have been with the same male for three years and have never had unprotected sexual intercourse. My last annual pap smear was September of 2007 and it was normal. The nurse practioner prescribed me birth control because to prevent any more possible cysts. I am just confused and would greatly appreciate any insight on what any of you might think caused this pain. Was it PID, a cyst, etc? Let me know if any of you have had any of these similar symptoms. My family doc says PID but the nurse from the specialist office says it was probably a cyst? Thank you for taking the time to read this and for any replies.

I have the same thing!

For me, the pain started a few hours after I had sex one day. I had had surgery before for a cyst, so I was already on birth control to prevent anymore. I normally have horribly painful periods, and knew this feeling wasn't cramps. It was a very sharp painful stab in one particular spot - just like you described. I went to a clinic doctor who did exactly what yours did - tried me for STIs, urinanalysis, put me on antibiotics. Nothing helped, all the results came back negative. I went to my obgyn who did an ultrasound and found NOTHING. I talked to my dermatologist about this because I have lupus, an autoimmune disease, and thought maybe it had something to do with it, but he said probably not. He did recommend that if antibiotics don't work, stop taking them. There is no reason to take something that doesn't seem like it's working, espeically an antibiotic (if they can't even find a bacteria that's causing it), and you also don't want to build up tolerance to a drug.

It is about five months later and the 'PID pain' comes and goes. I'd recommend naproxen, see if you can get a prescription for a higher dosage than OTC (Alieve). It doesn't work for me, however my mom swears by it (she takes it for cramps and migraines).

I would love to hear from anyone else, I am as stumped as you! Eighteen years old, been with this guy a year and a half and we BOTH tested negative for ALL the STD/STI tests that we've gone through MULITPLE times... =/
