Q: Pain from ovarian cysts versus mechanical back pain

I just had a lumbar mri and it revealed a disc protrusion affecting a nerve root and a an ovarian cyst was detected.What is most important to treat first, a disc protrusion of lower back or ovarian cyst? I keep having lower back pain on right side and really don't know what is actually back pain and what is the ovarian pain. Pain is going from front to back on lower pelvic area and goes around to my back, pain is also seriously painful in lower spine area and down my right leg and buttock area (I'm assuming that's the disc protrusion).
If the cyst ruptures, how will I be able to tell?
Any suggestions?

Your question involves which diagnosis is most important to treat, the ovarian cyst or the disc herniation.

Based on your symptoms, it is likely that both conditions may be producing somewhat overlapping symptoms. A disc protrusion, particularly if it is posterior right, can create nerve compression and cause pain down the back of your leg. Typically, the more compression is present, the farther down your leg the pain will go.

Pain from an ovarian cyst may produce the sensation of pain straight through to your back, but usually does not cause pain down the back and/or buttocks.

In these cases, it depends on the seriousness of either of these diagnoses, and the pain that is present. If the pain from the ovarian cyst is reflexively causing abdominal tightness, thus causing you to "slouch", then this postural position can significantly increase disc pressure, thus making your back pain worse from the disc protrusion. In this situation, it may be a good idea to address the ovarian cyst first.

One can tolerate "mechanical" back pain generally longer and with less risk than that of internal organ distress.
