Q: Pain during erection
asked by: DoctorQuestion on March 8th, 2009
Quite often when I get an erection it also comes with a cramp like pain in my pelvic and abdominal area. Sometimes the pain comes instantly and other times not at all. If I continue to masturbate eventually leads to a worsening cramp and some back pain. The cramp like pain only becomes a sharp pain when I ejaculate and returns to a worse cramp shortly after. The longer I draw out the ejaculation the worse the pain becomes.
Afterwards the "testicular cords" on one of my testicles become swelled and the testicle becomes somewhat tender. I have never had this pain anytime I didn't have or hadn't recently had an erection. And after ejaculation the pain eventually subsides completely.

I don't really do any really vigorous exercise like horse-riding, so I don't think it is torsion, but what could it be?

Any help appreciated

According to the data you reported, you might be experiencing pressure caused by some of the pelvic muscles. During erection, some of the pelvic muscles might involuntarily contract and thus put pressure on some of the surrounding organs. Since the contraction might be strongest during ejaculation, strongest pain is also expected at that moment. The location of the pain (pelvic and abdominal) might seem a little too far from the pelvic muscles, but please keep in mind that our body sometimes can change the perception of the location of the pain.
You might want to visit a urologist for your condition and have a ultrasound which might help visualize a potential cause.
Meanwhile, you might want to try changing the position of the body (especially the position of the legs towards the torso and each other), in order to try to relax the pelvic muscles.
