Q: Origins of shoulder pain

what could be causing this, it has been lasting for over 7 months now and appears to be getting worse.

It doesn’t seem likely to me that an insect bite could cause a shoulder pain that has been lasting for more than 7 months. According to your age you could be experiencing inflammation of the soft tissues around the shoulder joint or inflammation of the shoulder joints themselves. You could request an X-ray image of the shoulders to confirm such inflammation.

You could consult an orthopedist about your condition.
