What can cause a "notch" on a 16 year old girl's hymen? Can this happen without intercourse occuring?

Hi,
Welcome to ehealthforum,
Do you mean to say that you have found opening in hymen? Hymen can get ruptured even without vaginal intercourse (due to strenuous exercises, cycling, horse riding, masturbation, etc). There are also passages in hymen that allow for passage of menstrual blood and endometrial tissues during your period time. To confirm if hymen is intact or ruptured, visit your doctor/gynecologist for a thorough examination.
