These symptoms aren't specific, and may mean a simple case of constipation, or constipation linked with colon polyps or colon cancer.
They can be symptoms of irritable bowel syndrome, or diverticulosis.
If you are experiencing diarrhea-like stools with urgency, you may have proctitis or colitis, either the rectum, colon, or both, are inflamed and ulcerated (inflammatory bowel disease, bowel infection).
The rectal bleeding can be from hemorrhoids or anal tears due to constipation or chronic diarrhea, due to bleeding polyp or cancer, or due to damaged mucosa of bowel wall due to inflammatory disease.
Mucus and blood on stool are very characteristic for irritable bowel syndrome.
