Q: nausea and anxiety

I am a 32 year old male.
For many years I have suffered from being sick on the morning or night before "events", eg exams, going out for meals, work meetings etc. This include dry retching until tears come to my eyes if I haven't had anything to eat, which I generally don't as I know I'll be sick and can't see the point.
However recently I have had 2 knee operations and have been off work for two periods of time.
I am now back at work and feel substantially worse, and I'm begining to feel like this every morning.

I have previously seen my doctor, going back a couple of years and he gave me propranolol, which seemed to help slightly, but only if I took it in larger doses than proscribed. He says 1 10mg tablet 3 times a day, I have been taking 3 at any one time, but only when required i.e. I know I've got something coming up that will cause problems.

Anyone got any thoughts or can offer any help, I'm writing this at 6:30AM after being awake since 4:30, trying to avoid the cloud of dread that started to decend.
