Nausea, Abdominal Discomfort, Constant Belching
Posted: 06-14-08 19:03pm

I am a 42-year-old male. I am about 100 pounds overweight (and working on that).

I started experiencing nausea back in 12/07. It was assumed by myself and doctors that it was related to a long-term headache I was experiencing at the same time. The headache finally cleared up in April Shocked but the nausea did not. I started belching a lot too. This prompted me to see my doctor again.

He thought he felt an abdominal mass which he suspected to be a hernia and sent me for an abdominal CT scan. The CT scan (about two months ago) showed no hernia, all organs were normal, bowel was normal, no mass, no hernia. It did show a fatty liver and gallstones. The doctor thought the gallstones might be causing the nausea and recommended the gallbladder's removal.

I went to a gastroenterologist for a second opinion. He recommended an endoscopy and a HIDA scan and some blood work. The blood work and the HIDA scan were normal. The endoscopy showed esophagitis and minor gastritis. Nothing serious from the biopsies - no Barrett's, no ulcers, etc. I had been experiencing NO heartburn. He put me on Nexium, said the nausea and belching could be from reflux. And he recommended that I not have the gallbladder out. Then I saw the surgeon anyway, and he concurred with the gastroenterologist - no surgery at this time.

I've been on Nexium for seven weeks or so. I still experience nausea frequently daily (no vomiting) and now I have discomfort in the abdomen (about 2-3 inches above the navel in the center, sometimes mild pain, other times just noticeable discomfort) and I still belch like crazy.

No issues with stool, no vomiting, appetite is fine (strange with nausea, no?).

I'm sorry for the winding road my story is. It really happened this way - started out with the loooooong-term headache with nausea and ended up with abdominal discomfort and belching. The nausea is the one constant thing - I've wanted to vomit every day since before Christmas.

Thank you for any opinions.

Hello there doctor...

That was checked when I had the endoscopy. It was not present. That was about two months ago.

Thank you for your response. Any other advice?

Have you lost any weight?
Beside noticing fatty liver on CT did you checked the liver enzymes in the blood: AST, ALT, alcal phosphatase, gamma-GT, direct and indirect bilirubin etc.? How is your sedimentation rate and CRP? Can you tell me precisely which blood tests you have made?
Do you experience nausea after the meal or nausea is not related with the meals?
Is the nausea followed by dizziness?

I've lost a few pounds (intentionally).

Many things were checked with the blood work, but I don't remember liver enzymes being mentioned specifically. I will check that out and post again...

My nausea is not related to meals and there is no dizziness.

My gastroenterologist set me up just yesterday for a capsule endoscopy. That will be next week. He suspects possible inflammation (small intestine), and he also thinks it still may be related to abdominal wall weakness or hernia (though the CT scan two months ago did not show that).

Do you think I should be thinking in terms of a colonoscopy?

Thanks again.

Capsule endoscopy is used to visualize the small intestines because that is the only way. Large intestines can be visualized with colonoscopy.
Prolonged nausea is generally due to chronic disease, metabolic reasons or has psychiatric issues.

Well, I had the capsule endoscopy, and an interesting thing happened... The capsule took about 8 hours worth of pictures inside my stomach. This led my doctor to diagnose "gastropariesis". My stomach just empties very, very slowly. I had a colonoscopy just as a precaution, and that came back clean - no polyps or inflammation. So the doctor has me on promethazine, and this seems to manage the nausea adequately.

Thanks again for the replies... God bless from the USA.

I am glad that doctors found what really causes your digestive symptoms and that the therapy works.

Further to your problem.
I too have had a long bout of Gastro discomfort, constant bloating & Nausea.
It started in September 2000 & lasted about 3 months & then abruptly stopped for about 1 1/2 years before starting again. This time it lasted 3 years until November 2005 & then stopped again. It started again 3 months ago & is currently in high gear, I wake up with morning sickness (I am male). I have add upper & lower xrays, sigmoidoscopy, CT scan, ultrasounds, H-Pylori treatment, high level antibiotics,3 Endoscopies, colonoscopy & many blood tests without any conclusive diagnosis. The last time it stopped abruptly 2 1/2 years ago it was synonomous with my Colonoscopy. (I wonder if the pre op cleansing cleaned something out that reoccurs)
Any help would be appreciated.
