my father in law as myelofibrosis he can not have his spleen removed is there anything else that can be done or will his organs start to fail

Besides splenectomy, another therapies like androgens, chemotherapy, and splenic radiation therapy, can be given to patients with myelofibrosis, but only as palliative treatment.
The overall median survival is 5 years although it might range from 2 to 15 years, and poor prognostic factors are anemia, high or low white blood cells levels, low levels of platelets, older age and some cell changes seen under microscope.
Death is often due to leukaemic transformation, portal hypertension or infection.
