Q: material in urine

I'm not one for going to the doctors, (I rarely go because I can never get the time off work, and the practice is only open the same hours I work).

This all started back in February when I started having bowel problems (triggered by iron pills). at the same time as bowel pain I had bladder/ penis pain when going to the toilet. My bowel problem was later diagnosed as IBS, and when I spoke to the doctor about my bladder pain they said it was most likely to be a bladder infection. I was going on antibiotics (for acne) and so that should have cleared the bladder pain if it was an infection (in theory).

I still have pain in my penis when I go to the toilet, no longer in my bladder. My urine has a bad smell (smells like leather) all the time and has a slight orange discolouration. What has started happening in the past 6 weeks, is that some times when I go, I am passing some sort of material in my urine. In the urinal or toilet pan it actually looks similar to fine breadcrums, or there are larger bits that look like they could be pieces of dead skin. What is more, is that I also have been passing air through my penis when I go to the toilet, (much like you would through the back passage) which is also quite worrying.

Could someone give me some advice on what is happening here?

Have you ever noticed blood in your urine?
Do you have any urethral discharge especially in the underwear when you get up in the morning?
Do you have erectile disfunction?
Do you have often urge for peeing?
Have you noticed any changes in your sperm?
Can you explain that "passing air through my penis"?

Sorry for the delay.

I've never had blood for 100% sure in my urine, only sometimes a discolouration of an orange tint. I've never urinated out pure blood either.

I don't get any sort of discharge when I get up in the morning, the only sort of discharge is after I pee there is still a little bit that comes out afterwards once I am gone from the bathroom.

I don't have any erectile disfunction.

I get the urge for peeing a lot more often, maybe every 2-3 hours, or sometimes i can go longer without needing to go.

The only change I have noticed is that there seems to be a slight off-white discolouration. I would go as far as saying that it takes on a tint of the colour of my urine slightly. This is the only change.

The passing air part is like this; when I use the toilet I will be peeing and flowing normally, then all of a sudden the flow will stop and air along with a little bit of urine will be coming out. Because it is urine and air at the same time, it bubbles up when it comes out, and when air comes out on it's own it sounds like passing wind. The urine that is basically dripping out along with the air is very dark and contains material.
An hour or so before I need to go, if I am going to pass air through my penis I know it's going to happen because I get a pain at the joint of my penis and right behind the penis inside.
This passing air may happen every time for a day and a half when I go to the toilet but then I can go about 5 days peeing normally.

According to your telling you should consult some urologist. You may need to do digital rectal examination and cystoscopy. Blood and urine laboratory tests will be also requested.
