Left testicle painful and loose

Hello There ,I read your information posted on your website regarding testicle problems. For quite sometime I m having problem with my left testicle. I m having it for last 7 to 8 months. First I think I would give you a brief information about it.
I stayed in philippines for almost 10 months. I used to masterbate a lot there and had on occasion have unprotected sex just 3 or 4 times. It happend 3 or 5 times that i masterbated for 7 or 8 times in a row. And then after 4 or 5th time I noticed there was no sperm flow at all only a drop.
I was drinking and smoking a lot those days ..When I would have sex .. after drinking beer or whiskey i would have on viagra tablet.. which would give me a kick and i would keep doing a girl for 40 mins or more then that. I remember last doing it in july 2008 i done a girk for 40 mins continously after i ejaculated I felt inflamation in my left testicle. which i felt next day as well. Time went by and I didnt care much i thought it could be bcos i did a girl so much may be could be a small thing.
Then In july 2008 i came back to india. I kept on with my masterbation thing. I would masterbate 3 or 4 times. One day I wore a very tight underwear which is very small for my size and I went for jogging. After i came from jogging I felt a mild pain stretching from my left testicle to my left hand side abdomen. This pain continued it would be there and next moment i may not be.
I could feel a stretch from my left testicle to the left hand side of my abdomen.
Then one day I felt a swelling of a muscle over my left pelvic girdle bone. just 1 inch over that bone. It was as if a half by one inch long pipe have been inserted in my flesh.
And my left testicle would hand few millimeters more then the right one. and if it touch the base of the sack i would feel a pointed protrusion(testicle protrude from base) due to may be cuz of swelling.
then i showed to general practitioner. he examined he said it is infection and gave me few medicine. which i dint take for long.
Now its been 8 months when i first started having that swelling in my left girdle which was gone after two months.
But now when I masturbate I notice my erection is okay but when i ejaculate my sperms dont go far off they just jump to one inch . naturally or penis shoots jumps far off.
And I notice the veins which connect my left testicle are thick then the veins of my right testicle.
And my left testicle still hang longer when the sack is soft but when the testicle sack or scrotum is stiff they r okay.. and both r equal...
In morning I have noticed I dont have the hard erection when i would get up as I would used to get earliear.
The only heart problem i had is below...

And 4 months ago I have 208 calosterol level which on american medical website is a boderline risk. I smoke excessively and i had a pain in my left hand and finger were numb this is the heart problem i had. but now its fine cuz i have cut down on cigarettes.

Plz doctors help me. Over here we dont have that kind of infrastructure and medical help unless and until we exactly know what the problem is. I wonder if anyone would identify the problem ..U must be having contacts and u can get me the right information I will be waiting for the reply..
