Itchy nipple/pain around breast for months

Hi, Can anyone tell me if they have had the same problem as myself please.
I have had for months now a very itchy right nipple, it drives me crazy sometimes and it will last for days and then subside but it returns.
In the same breast on the left side it hurts and for the past week pain has occured at the bottom of it.
I am 37 and have taken HRT since the age of 23 after a premature hysterectomy, could this be the reason of the breast problem?
