Q: ill after trip

Hi, just short of 2 days in Ireland visit(so not sure if related) 18 y/o son had chills fatigue, nausea, loose stool (not bad diarrhea), dilute urinary frequency on July 9th. Had gone to a Seaweed bath house and bathed in raw hot seaweed that same am. Has seemed to get better x 7 times then has another bout of mostly nausea/fullness and never feeling completley well...loosing wt., pale. No fever known. No hx. of G.I.prob.. Has taken course of Alinia, then 5 days Bactrim, and currently takes Aciphex 1xday. Lab unremarkable, bilirubin elev, told Bili is Gilberts syndrome. Neg stool for Giardia ant. x1, crpto ant.x1, o&p x2 -3. Was prescripted Tindamax but we are hesitant since he already took Alinia? what a way to start his senior year! Need some direction please. thank you. Regards, Vicki

Doctor
How many times per day he has diarrhea?

Is he vomiting or only has nausea?

Is there mucos or blood in the stool?

Keep in mind he's still a teen and not always communicative. Turns out it wasn't really diarrhea, just looser stools, and not very many. Maybe small mucous, no blood though. His main symptoms are Nausea and "burning" like in the umbilical area. It comes and goes or "spikes" , from a low level 1-3 then spikes to ~8 on a scale of 1-10. the spikes are 1-2 times a week generally. can't connect to what he ate too much. Has been on a bland diet during this ~7 week period. I realized we have no Pancreatic enzymes and now I am scared because his blood sugar is at top of normal, told he has Gilberts syndrome (is it really I'm thinking), nausea, dyspepsia? so I am worried about Pancreatic or duct ....tumor. Gilbert's syndrome has nothing incommon with the pancreatic enzymes and blood sugar level. Gilbert's syndrome is due to a genetic defect that causes specific enzyme deficit in the liver cells. The lack of the specific enzyme makes the liver unable to transform unconjugated bilirubin into conjugated. Thus the unconjugated bilirubin in the blood rises and that causes jaundance. Gilbert's syndrome can be diagnosed when other reasons for jaundance will be excluded.
Does your son have jaundance?

Can you be more precise about its pancreatic enzymes? How do you know that pancreatic enzymes are missing and from where they are missing?

I don't know if your still available. If so, I would appreciate any fresh ideas on what to ask of doctors here. We are at the Rochester Mayo clinic. My son is loosing too much weight, found to have Gastroparesis. Reasons unknown, guessing from a bug. but which is why we are here. Worry that Irish bath was cause last year, maybe hidden parasite??? or Chagas from all the S.A. travel we have done. Stools negative, which doesn't surprise me. Had EGD today, consult with infectious disease per my request in am. If the wt. loss doesn't stop I fear he will end up with feeding tube. thanks for your responses. Worried mom
