Q: Hypertension medication taken irregularly

A nurse-practitioner prescribed Triamterene/hctz 25/50 mg with a "take as needed" note on the prescription for my husband. Since then at two other medical appointments with specialists, nurses have told him he should take the prescription daily, not "as needed". His b.p. has been rather high for years -- 158/94 is not uncommon for him-- and was told at his health fair at work to see his family dr. His feet and ankles swell regularly. His mother had severe hypertension for decades and had two strokes. My husband only wants to take the medicine when his ankles swell or his b.p. seems high. He has been taking his b.p. daily to "prove" he can just take it irregularly. He works 12 hour days most days and a few times a month he works 18-20 hours/day, volunteers about 12 hours a week and sleeps about 5 hrs./night. I was so relieved when he started on the medicine but taking it irregularly concerns me. Please advise.
