What should a wife do in this case pleading begging and rational conversation has not worked. He is in denile about taking his insulin

What type is he? Unfortunately, there are some things he will have to learn himself. There is a lot to deal with when you have Diabetes. How long since he has been diagnosed? Is he seeing a specialist or just a GP?
