Q: HIV risk?

Hi Doctor,

I have a specific incident on which I wanted to ask your professional opinion. I am currently traveling in Mexico and did something that I am mentally stressed and deeply regretting exactly one week ago....had protected sex with a stripper for money. One thing that scares me is that 5 days prior to this incident I went to a doctor in the states for a lesion/abrasion on the head of my penis. I shouldn´t have been at risk for an STD prior and the doctor did not think it was Herpes, but I dont know the results. Anyhow, my major concern with the episode in the strip club is that I had sex while wearing my boxers and after the incident I pulled my pants up (condom still on) and went in to the bathroom where I carefully removed the condom and put hand sanitizer (ethyl alcohol) on my penis and scrotum. There must have been vaginal fluid all over the boxers and not thinking I pulled them back up and my jeans as well. My fear is that the abrasion/lesion was probably exposed to vaginal fluid shortly after sex (2 minutes max)....does this put me at a great risk for HIV transmission?

On a side note, I am kind of a worry wart and had a slightly sore throat prior to the event, the throat has not improved at all (actually feels a little worse), lymph nodes are swollen in the neck, I´ve had night sweats and the scariest thing is that I have noticed about 15 small red circular dots in different parts of my body (hands, arms, shoulder, feet, upper chest and shoulders). The only physical symptom that really scares me are the red dots, I have never had those in my life.

Thanks for looking at this post and to restate, my questions are really-

Is this a high risk incident?
Would ethyl alcohol (hand sanitizer) help create a barrier to entry if HIV was present in the fluid or would it help it enter?
Could these red dots on my skin be something else?
This abrasion (on the head of my penis), if it is not herpes, does it still give a good entry point for the HIV virus?

OK to start off I am not a doctor but this is my opinion.

No as long as you wore a condom the whole time, including oral sex, then your not at high risk. This is because HIV can only live outside the body for about 30 seconds, maybe a minute, so if there was HIV on your boxers and it touched your penis it was probably already dead. As far as I know ethyl alcohol doesn't provide an effective barrier. About your red dots, I personally think its stress and nothing else. Yes other STD's, including herpes, can allow an easy passage way for HIV to get through but since you wore a condom then its not that big of a deal. Remember when you get back home get tested. It's advised to wait 3 months before getting tested but about 95% of people will test positive within 6 weeks and 99% within 3 months but you still have to get tested 6 months after your exposure too.
