Since the symptoms reoccur when the drug is withdrawn, you should be prescribed the stronger dose of the same medicine.
Even when symptoms are completely relieved by medication, they usually return within a few months after drug treatment has stopped. Long-term maintenance may be necessary.
When life style changes and drug treatment aren't helpful, a surgery is usually recommended.

Have you been to a gastroenterologist and had your esophagus checked out? You said there is scaring. Did they check for precancerous cells? I've heard that when they take those out, they can also do a surgical procedure that fixes the heartburn. Talk to your doctor about it.

You also need to stay on your medication, rather than periodically going off of it to see if you still need it like you are doing now. It's not going to heal on it's own if it hasn't already. Also, many of these stronger medications need to build up in your system in order to work properly.
