Q: Headache increases when lying down

I am asking this about my husband. He has a history of uncontrolled HTN, Slightly elevated cholesterol panel, and Fasting glucose of 122 and overweight. He has had a temporal headache for about a week now, right sided pain increases when lying down. Also experiencing dyplopia, binocular, horizontal. He states this is worse in the evening. Denies any other deficits. I am an NP so I did a very limited neuro exam and found CN II-XII to be grossly intact. His BP had been running about 150 systolic and 90-110 diastolic. Also to mention he is one of those noncompliant patients even though I stress the importance and given the fact he just informed of this yesturday and refused to go the er when I wanted to take him. What are the chances this could be something of an emergent nature? My differential is broad including thing like temporal arteritis through to a bleed. Any insight?
