When foreskin of the penis is pulled back, it can constrict the glans penis if the foreskin is tight, resulting in swelling of head. Skin can also get swollen. This can restrict blood supply leading to problems. He needs to be shown to the doctor immediately. It can be corrected conservatively by reducing the swelling with use of some agents or needle puncture if the constriction is minimal. If this doesn't work, a circumcision may be needed.
