From the description provided it seems that your fiancee has severe chronic constipation which has resulted in a fecal impaction, which might have caused intestinal obstruction symptoms.

Since even after an enema, the impacted stools have not been cleared, she might benefit by using DULCOLAX (stool softeners) to help with relieving her symptoms.

In case this does not work, and her symptoms worsen, she might need surgical interventions to help her.

You might consult a gastroenterologist at the earliest for resolution of her symptoms.
