Q: Enlarged lymph nodes affecting breathing ?

I had pleurisy. Boyfriend's been sick since January and treated with 3 courses of antibiotics, but still sick, pain in both lungs, becoming more difficult to breathe each day and is affecting his heart. He cannot walk down hall without losing wind and stopping to rest. Doctor ordered CT scan last week after radiology report revealed enlarged lymph nodes at site of right lung breathing tube. Since then it is affecting his breathing worse and heart rate. Can the enlarged lymph nodes cut off his breathing? He is being stubborn as we have no insurance or money for medical. What do I do for him? Two days ago he awoke from sleeping with pins and needles pain sensation across his chest. What is going on?
