Doctor refusing treatment? first stage

Hi,
My Mom was diagnosed with the first (lowest) stage of lung cancer 4 months ago. She received 8 radiation treatments. We just got another CT scan done showing that there is still cancer in her lungs.
****My question is can her doctor refuse to provide further treatment because she is unable to quit smoking?***
Please keep in mind she is 64, has been smoking since she was 13. She also has been diagnosed with depression that has lasted for the last 18 years. She has and is continuing to be treated for the depression.
We do understand that she should quit smoking and she has tried all the different methods, however she is old and with the constant depression she has been unable to quit.
