Q: Discoid lupus
asked by: DoctorQuestion on October 9th, 2009
I have had sub. discoid lupus diagnosed thru biopsy since 2004...had it much longer...I have 5 of the symptoms for sle and heart, and lung problems...I always have a neg ANA. I was referred to a research study where my ANA was still negative so I didnt qualify but the Lupus Dr. said he would see me at his practice. They took 22 vials of blood for the screening. He said he is extremely confident that all my symptoms are related to lupus. He is seeing me in Nov. and switching me to antimalaria meds as on steriods my discoid rash is still severe. In the testing I tested positive for TB Blood test and am in the process of gathering sputnum for my health dept. If I have to make meds will it effect my lupus and lupus meds? Could I have a false positive? They are also giving me the skin test for tb today. I'm scared

Hi
Welcome to the forum!
TB blood test is rarely false positive. It detects either a latent infection or an active one. This is done basically to overcome the false positive or false negative result of TB skin test. You would be taking the skin test too so if this too is positive then TB is confirmed.
Regarding ANA being positive: only 20% patients with discoid lupus are ANA positive, so it is not a major diagnostic criteria. Biopsy was confirmatory in your case.
In discoid lupus patients with negative ANA, the possibility of another systemic illness like tuberculosis should be suspected, especially if there are other symptoms apart from skin lesions, as mentioned by you (symptoms for SLE and heart, and lung problems). If at all you have TB then you will have to take treatment for the same. It is possible to treat both together.
Hope this helps. Take care!
