Q: diabetic tests

Hi,

I'm a medical student and currently i am revising diabetes. I was told of a question we got that had a list of tests and we had to match them to whether it was testing glucose management over the last 3 months, the last 3 hours, if they were a diabetic, current glucose management and also if they had peripheral oedema what test would you perform

I want to check that the last 3 months would be HBa1C?
the last 3 hours would be urine test of glucose?
the currnet management would be blood glucose?
to diagnose for diabetes would be fasting blood glucose?
finally for oedema i said check their blood pressure on the theory that a high blood pressure would force more filtrate out at capillaries causing oedema but this was incorrect so could it be albuminurea as this could cause a decreased colloid osmotic pressure in the blood thus causing less water to be reabsorbed and thus causing peripheral oedema?

Um...I'm a diabetic, and this is how I would answer...

Check if they are diabetc: Urine (that's how I was tested)
Check last three hours: Blood test (glucose level)
Check last three moneths: HBa1C

Current glucose management? The doc checks my log book and HBa1C for that.

That last one...I don't even know!

I know the GTT is used to diagnose diabetes here, although the fasting blood sugar can also be used first.

Current management I would have thought would be a log book. Although my husband (type 1 diabetic) has regular blood glucose tests to check how he's doing, so that could be what you're looking for.

The HBa1C would test your average over the last 3 months yes.

Would a visual examination diagnose edema? That's a shot in the dark.

A urine test I would have thought would just be for ketones, high blood sugar. Not sure about that one. It could indeed tell the last 3 hours.


Just noticed you're from Minnesota Syndey, that's where my husband is from Very Happy

Wow- he sure did travel a long way from MN to the UK! Good for him...heh.

Anyways, it looks like checking blood pressure would be for the edema. I checked a couple places online to check. :: shrugs ::

It does look like it's blood pressure, I don't see why that would be 'wrong'. Unless they're looking for a physical examination. I don't see how any kind of blood sugar test would help...

i think that the current glucose question was referring to the glucose levels in the patient when they walked in there and then which i assume would be a blood test?
but then i get confused how it can also be for 3 hours and not sure which one is right!

i think it meant if there is a diabetic with oedema what do you need to test for - because in a diabetic it is likely to have diff causes - it was a horrible exam though and hence why i am resitting!

Well I can only speak for how things are here. But a current test would be a blood glucose test. For 3 hours it would have to be urine as a general blood glucose test wouldn't give you that information.

Edema to be honest I'm not too familiar with. Even my husband isn't 100% and he's been diabetic all his life!
