Q: decifering MRI report of spine

I have a close friend in the hospital with intractable back pain and they have done a MRI of her spine. I am having trouble deciphering the results, and the doc's say that there is nothing they can do for her pain. The results are (abnormal's only):

Cervical spine- Small focus of signal abnormality of the superior C4 vertebral body near the end plate likely represents focal fatty marrow replacement. There is a small focus of focal fatty marrow replacement superiorly at T2. There are very minimal bulges at C4-5, and C6-7 which do not result in significant marrow compromise. IMPRESSION: no significant compromise or cord signal abnormalities.

Thoracic spine- small focus of fatty marrow replacement superiorly of T6 at the end plate. IMPRESSION: same as above

Lumbar spine- Posterior annular fissure at L4-5. There is disc dessication with mild narrowing of the L4-5 and L5-S1 intervertebral disc spaces. There are hemangiomas or foci of fatty marrow replacement of L3 and superiorly of L5. There is focal protrusion and left paracentral extrusion at L5-S1 which contacts the ventral surface of the descending left S1 nerve. Mild narrowing of the left neural foramen at L5-S1. Minimal protrusion at L4-5. IMPRESSION: Left paracentral disc extrusion at L5-S1, which lightly contacts the ventral surface of the descending left S1 nerve.

Not only is she in severe pain, but she is having trouble urinating and walking. Her left leg keeps giving out. She has minimal nerve reflexes in bilat. legs. She has been battling back pain for 5 years now, undergone the back injections, with no relief and doesn't know what to do. Tell me what you think! I would like to help relieve her pain.

hi cricketke sorry but these findings need to be correlated with other clinical exams to have meaning if you talk to her doctors ask firmly and in a calm manner that they tell you in detail to the best of thier ability what is wrong with your friend

Hi Cricketke,

I to am sorry about the pain that your friend in.

You need to discuss the results of your friend's MRI directly with her spine surgeon.
Ask him/her to explain what the results mean, and what his /her interpretation is of the MRI as well.
And, also ask why her doctor why he/she says there is nothing that can be done for her pain.

I would also be sure that the spine surgeon is able to read an MRI and is not just relying on the information that is provided with the MRI report that was read by a radiologist specially trained in reading MRIs.

Another important thing to remember, as well, is that the interpretation of the MRI by the radiologist is just that, his/her interpretation.
The spine surgeon's interpretation, if he/she can read the MRI, is also just that, his/her interpretation.
It is not necessarily correct.

It is always good to have more than one opinion from a spine surgeon, especially if surgery is suggested or if you can not get treatment that helps from the spine surgeon that is being used.

If you get no satisfaction in your answers from your friend's spinal surgeon, or no treatment and/or pain relief is offered, it would be in your friend's best interest to seek out another spine surgeon for another opinion.
You may need to see several spine surgeons before she finds one that can help her.
