The anal mucosa is very sensitive. If you have constipation, the feces would be more solid and it might hurt the anal mucosa when wiping. This would be the least dangerous diagnosis. Taking food rich in insoluble fibers would increase the volume of the feces, thus helping to solve the constipation.


However, other more serious conditions have to be ruled out before. You might want to visit a proctologist to check for hemorrhoids. If none can be found, additional examinations might be required to discover the source of bleeding. Even if the bleeding is not visible, it can still be detected with an Occult Blood in Feces Test.


About the occasional itching in the right breast, you might want to do a mammography on a regular basis, having in mind that your sister has a history of breast cancer.
