Q: Constant bad breath treatment

I have had breath. Specifically, when I wipe my mouth with my hand and smell it, it's quite bad. My mouth sometimes gets a white ring around it. I'm guessing this is from dryness. It seems that the bad smell is coming from outside the mouth.

How do I fix this? I floss, brush, use mouthwash, and clean my tongue. I'm fine for a few minutes then it's back again.

I also drink lots of water.
