Hi,

Well, I'm a 17 year old female and have had this pain for a little over 3 months. It's a consistent sharp pain in my lower right abdomen, that is consistently there. I've been admitted to hospital 3 times, and been to my doctors several times. I've had X-Rays, Ultrasound scans, an MRI scan, blood tests, and everything is showing up clear and perfectly normal.. but the pain isn't getting any better, but gradually worsening.

My first attack 3 months ago proved excruciating. I couldn't sleep at night, and in this rare case I basically cried myself to sleep in agony. The pain was severe and sharp in the lower right abdomen, however, this came on without warning. I was consistently feeling nauseous, and suffered when walking, and eating in real bad pain. I was admitted into hospital for 4 days for observation, and then released after being on IV fluids, and told to take paracetamol, and tramadol, and return if the pain worsened.

The pain never left, but subsided to be what I call just about bearable. A few weeks later, I had another attack when I was in college. As I study a horse care course, I was currently riding when it happened. In the matter of moments, I went really weak and light headed, and extremely dizzy. My entire mouth and throat went dry, and I started struggling to breathe. The pain in my lower right abdomen worsened and was severe. The pain was extremely sharp, and my abdomen extremely painful to the touch. My oxygen levels dropped from 99% to 93-94% in less than an hour, I nearly collapsed, felt extremely weak and nauseous, and even lost the ability to move for a while. Still the pain in my abdomen remained, and I was shaking pretty badly, from the pain. Again, I was admitted into hospital, for a further 2-3 days, put on IV fluids, and then given blood tests which showed elevated white blood cells. I was also give X-rays and an Ultrasound scan (of the pelvic region), which proved to be normal. They suspected appendicitis or ovarian cysts that were possibly ruptured, but passed them off due to the tests being clear.

Again, the pain subsided to be just about bearable, once again. I was coping just about, however the pain was there, and made me feel uncomfortable often. But this lasted for around a month, until I had my last attack around 2 weeks ago. It was around 4-5am, and I was awake, sitting down and talking to a few friends on my computer. Suddenly I had sharp pains in my lower right abdomen, which I have never felt anything like before. It made me keel over in utter agony, and brought me to tears. I had really bad pain just above and below my navel, that when touched felt like an excruciating 'burning' sensation. Later that day, around mid-day, the pain worsened. I couldn't walk, and when I tried, I nearly collapsed. I felt extremely week and nauseous, and really light-headed/dizzy. I was in tears from the pain. This happened on the Wednesday, by Friday, I went to my doctor, and was sent to hospital with suspected acute appendicitis. II spent a further 8 days in hospital. I had also lost my appetite, and never ate for 4 and 1/2 days. Eating, drinking and movement proved to worsen the pain, as did lying flat on my back. I had blood tests, X-rays, an Ultrasound scan of my entire abdomen and sides, swabs, and all came back clear. A week prior to this I had an MRI scan of my bowels, and this also proved clear and normal. However, when the surgeon examined my lower right abdomen, it was extremely tender and I began guarding when touched. When pressed on the lower left abdomen, it referred pain to the right lower abdomen. My lower right abdomen was tender, and slightly swollen, most specifically over 'McBurney's Point'. He then suggested it was my gallbladder, but proven wrong when the tests came back clear.

A few days ago, I was to have a laparoscopy and possibly an appendectomy, but then the next day, dismissed from the operation and released from hospital, to be told to again, take paracetamol and tramadol, and return if the pain worsened.

I'm so confused on this, I've had blood tests, swabs, X-rays, Ultrasound scans, an MRI scan, and all are proving to be clear? Yet my second attack proved to have elevated white blood cells? But nothing since. I've never had anything like this before these past 3 months. The pain seemingly came from nowhere, and now I'm having regular attacks, which result in nausea, excruciating lower right abdominal pain, lower back pain, pain occasional felt around the navel, and a little left of my right hip bone. I've also had bouts of chills and slight fever, and I find I'm shaking a lot. I frequently lose my appetite, and find that I don't eat several days on end as eating/drinking and movement worsens the pain. Again, the pain is still present as a sharp pain in my lower right abdomen and is consistent. The medication fails to subside the pain, and does not affect either of me, or the pain...

I'm clueless, should I give up or keep looking for answers? Has anyone else had any similar problems to this, or have an answer to what it may be?

Many Thanks;

Nicole-Louise.

Hi,
Welcome to the ehealthforum and I am glad to help you out. From your symptoms and the tests coming out negative for everything, it can be due to acute appendicitis, mesenteric ischemia, endometriosis, omental torsion or ectopic pregnancy.Symptoms of endometriosis include pelvic pain(usually before menstruation but sometimes during intercourse, urination or defecation), sometimes diarrhea or constipation, irregular or heavy menstrual bleeding, sometimes blood in the urine and inability to conceive.

Although ultrasound is useful in diagnosing endometriosis, but still cannot definitively diagnose endometriosis.Laproscopy is the only accurate way of diagnosing endometriosis.

I suggest you to discuss these possibilities with your physician. I sincerely hope that helps. Take care.

Thank you. It does help sincerely. But seems as if appendicitis is coming up too often, and the symptoms and specific pain is in that area, a bit too coincidental...

I was wondering though, can pain that has lasted over three months, subsided and flared up be appendicitis? I've been told about chronic appendicitis, but is that all too rare as they say?

Thanks.

Hi,My Partner is going through your exact problems described. about 29 motns ago we were trying for a baby with no sucess and she was going through these pains and had been admitted to hospital 3 times with it. The pain would come on so intense she would drop to her knees and curl over and some times faint. They did all the same tests (ultrasounds, bloods ...) and put it down to appendicitis, she had surgery and had her appendix taken out and they said she had some adhesions which they removed. All was well and within a couple months Kylie became preegnant and had no more pains. Our son was born and around 14 months later the pains are back. She has got to a point where she could not handle them and has been into hospital 2 weeks ago where they did a MRI and all came back normal, then a week later she had to go back in and they now think maybe intestines or some kinda nerve causing the pain. I would love to know how you got on with your pains and if you have found an answer. My poor parttner is just over it and its really getting her down.

It sounds like endometriosis. You should see an obgyn and see what they think. Unexplained pain is no fun - I'm going through it right now hoping to get a laparoscopy done soon.

LOWER RIGHT ABDOMINAL PAIN
This absolutely sounds like endometriosis, get a laparoscopy. I have been suffering with these unknown pains for about a year now and have been sent home from ER and given Panadol. The pains have slowly gotten worse. I wish I knew what it was sooner, but it WAS endo. No fun at all.

I've been suffering with the same awful pain and symptoms forever now. I had adhesions removed then my right over - at which time they said I developed endometriosis. I've also been told there's a good chance I have adnometriosis (which is basically endo but inside the uterus). I am so tired of being sent to the ER to rule out appendicitis. I'm going through an awful case of it now and with no ovary they have nothing to blame it on. When my doctor originally removed the adhesions and ovarian cysts, he said the adhesions (scar tissue) was wrapped around the appendix. I'm seriously wondering if there isn't nerve damage or something to the appendix. Unfortunately, I am unable to get any help. The hospitals here do not care how much pain you're in, if the test are normal, they send you on your way. I'm ready to suffer forever at this point.
