Q: Conception with severe dyspareunia ?

I have experienced Dispareunia since I first had intercourse. No cause has been found, but I suspect it has something to do with an accident in grade school where I fell off the monkey bars and landed with another bar between my legs. My first partner was smaller, so though there was pain, it was pleasurable enough to enjoy it. My husband, however, is rather large and I have never really been able to enjoy it with him. We have talked about it, and though he is disappointed, he understands. He does not like to cause me pain and my discomfort is obvious. I would say that penetration is the most pain I have ever felt, enough to bring tears to my eyes. But over time, this problem has affected my sex drive. I am at the point where I do not desire even the alternatives we have found to sex. We have been together eight years now, and are ready to have children. My doctor prescribed me this cream that was supposed to relax my muscles so I could enjoy sex, but instead of relaxing me, it kept my husband from reaching ejaculation. So instead of just being painful for a few minutes, once the pleasure for me was over, it was another ten minutes of pain before I just gave up. I fear this experience has made things even worse. I am in despair. How am I to get around this problem to conceive, and once I do will this problem make the pain of childbirth unbearable? I dream of a peaceful birth in my own home, but now I am doubting my abilities to do this. The only other suggestion my doctor has is a botox injection, which would make that area numb, but it would also keep me from feeling anything at all. Please if anyone can help me you would have my eternal gratitude.
