Full description of the patient while admitted in hospital:

patient has been taken to hospital in deep coma state after the attenders have found him unconscious at home(2 days duration).details unavailable O/E patient unconscious E2,V1,M1 pupils (L)-cat (R) Dilated no RL.

IN HOSPITAL AFTER ADMITTED:

ICU care with ventilator support.Tracheotomy has been performed,PEG procedure done and physiotherapy

ON DISCHARGE TIME:

Patient semi-conscious. Eye opening spont-E4 V2 M3 vitals stable.Tracheotomy closed.PEG tube in,foleys cathetre in.

MRI Brain :Acute infarcts involving (L)cerebellar hemisphere and pons,small chronle (R)MLA-PCA water shed zone infarct.
