Q: Chest pain , nausea ?

I've seen many drs and no one can give me an idea what is causing my symptoms. I know you can't tell me what I have, but some ideas of a direction to pursue would be very helpful.

I'm 34F with no risk factors for cardiac problems. I am 5'6" and 128lb. I work in a call center for 12h shifts, but am very active on days off. Over 2 mths, I've had increasing SOB, dizziness/lightheadedness, nausea and L chest pain extending into shoulder on exertion and occasionally at rest. ESR, D-dimer and vit B12 are elevated. ECG 1 mth ago showed HR 49 with "atrial anomaly and atrial deviation." BP is normally 110/60, but recently is ~90/~60 with frequent dips <90. Postural hypotension frequently. Rushed into ER 2 weeks ago for chest pain, presyncope, diaphoresis, hypotension and nausea. Pain relieved with nitro spray. HR in ER for hours 45-55. ECG sinus brady. Stress test and holter showed change in foci with atrial tachy (~166), but dr saying that changes in ecg do not correlate with symptoms. Echo showed trace pericardial effusion.CT angio showed mild linear density in left lower lobe, L basal subsegmental atelectasis, and moderate pectus excavatum, causing deformity of R heart chambers. During MPI resting HR 63, standing HR 96. BP 100/75. Stress images showed decreased activity in anteroseptal region, with no reversibility at rest. EF 65%. Defect was attributed to attenuation artifact.

No diagnosis has been given to me and thus no treatment. I am dysfunctional in ADLs and cannot work. Any ideas of what could be happening and what type of treatment I should pursue? Thanks for your help!
