Q: Chest pain , jaw pain , arm pain

Let me start off by saying I am male, age 29 and smoker. I am close to the correct weight for my height - 5'10, 175. I have been having pain in my chest around my heart area and pains in my arms in many places. I have jaw pain synonymous with the chest pains sometimes, but most of the time in just one of the three places - chest, jaw, or arms. Recently I realized that big meals trigger my heart to race and I feel like I am about to pass out from rapid heartbeat and pain in my chest. In the past couple of years I have experienced many different problems. I had swollen glands in my neck and diarrhea that lasted for a month. I got tested for HIV and was negative. I have had chronic UTI, but tested neg. for anemia. I have developed digestion issues, and only have one bowel movement a week. I have no energy, and if I drink any caffeine my heart races and I can't sleep. My legs are weak and it is hard to go up or down stairs. About three and a half years ago I was poisoned from home insecticides and it seems that is when all of these issues started slowly arising. Family history includes heart disease. Please help
