Given the symptoms you reported (pain in testicles, swollen structure in the scrotum), it is possible that you might be experiencing varicocele.
Varicocele are enlarged veins responsible for testicle drainage. The cause might not be identified in the case of idiopathic varicocele, whereas in secondary versions, varicocele are due to compression effecting the vein drainage. The symptoms usually include: scrotal pain, feeling of structure in the scrotum (sometimes described like bag with worms). The pain might be increased when the abdominal pressure is increased (coughing, sneezing, trying to defecate, lifting weight) because more blood stays in the varicocele, thus dilating them and causing more pain.
You might want to visit a urologist for a physical examination. An ultrasound examination might be needed to finally confirm varicocele. Surgery might be recommended to treat the varicocele.
