Q: Carpal tunnel syndrome and non-surgical treatment

Dear Sir,
I just turned 27 two days ago, and the symptoms of CTS got even worse. I have it in my right hand.

The severe pain started in Aug 2007, the severity decreased later on, but I was never able to get rid of it.

My question is that how long I need to wait before I will see an improvement (emplying that I am going in the positive direction). The nurse told me to wait and look for improvement for the next 4 weeks, and then go to the doctor, but I also do not want it to get worse.

If I need a checkup, should I go to a doctor or orthopedic surgeon?

I will really really appreciate your help. I thought I am too young to get into these problems :(.

List of other diseases, conditions, or injuries and their treatment: I have had a sever painful injurt in my right wrist 2 years ago. Did not go to the doctor, but I was in pain for weeks. Do a lot of touch typing. Was not having an ergonomic posture. Ride sports motorcycle.
Have a diagnosis for the current condition: no
Current medical status: CTS and Alnur Tunnel Syndrome
Current medical treatment: Bangay cream at night for the last 2 nights, 1400 mg of ibuprofen every day for the last 5 days, Braces almost everytime for the last 3 weeks, Switched to ergonomic keyboard, Using left hand for the mouse. Almost no motorcycle riding

Mild cases of CTS can be treated with non-surgical treatment (immobilizing braces, osteopathy, physiotherapy, chiropractic, massage therapy, medications, local injections, prioritizing hand activities, ergonomics, etc.). Therapy depends upon the cause of CTS. I agree that 4 weeks is a long enough period to start feeling improvement from the non-surgical treatment. First, you can consult the doctor who prescribed the non-surgical treatment. He should evaluate the results from the therapy and then make a decision if he is going to refer you to an orthopedist.
