Q: broken T11 treatment ?

HI ALL,

my son broke his back the T11 with some compression on 1 aug 09, he is out of his brace and to look at him all is ok. However he is still in pain and manages only whilst he continues his medication-tramadol/panadol with some dyclophenic as an alternative to the tramadol which he doesnt like. He has tried several times to go without tramadol but doesnt get any sleep. We have been referred to a PT with the thought that the ongoing pain is due to muscle wastage. We have also been given a number of different time frames for recovery from 9-12 months. My question is, what actually should we be doing at this point? Should we be requesting a MRI scan?should we be seeing a pain management team? should we proceed with the PT and play the wait and see game? My son has had to pull out of Uni and I dont want him to be the 'man with the bad back' from the age of 19, nor addicted to pain killers! thanks to all who reply.

Its a really good question I think you have a good understanding of the predicament you are in if you got an MRI it may show nothing even though your son still has a problem, and if it reveals somthing it may not be the cause of his pain or the doctors may still want to continue with the phsio therepy. An MRI itself could not do any harm an ill advised surgery resulting from its findings could. All i can say only deal with really good doctors

Though an MRI wouldn't hurt, as roller said, in the wrong hands it is not good.

I would start with the PT, make sure the physical therapist is well trained in spine conditions on all levels and let you son have the physical therapy evaluation.

Once that evaluation is done, get the opinion from the physical therapist as to what he/she believes is going on and what that opinion is for decreasing the pain and full recovery.

It very well could be that because your son hasn't been using those muscles for some time now, that they are very weak and weak muscles can cause pain, increase pain and be the cause of what your son is feeling.

Once those muscles have increased strength, you should be able to get a good idea if the PT is decreasing his pain and there is steady improvement.

If the PT doesn't think that she/he can help or PT will help much, OR if after several weeks there is no improvement, I would then go back to the spine surgeon and request a further evaluation, MRI and see what shows.

IF for some reason, surgery is suggested at that point, then it is time to get several other opinions before heading in the direction of surgery.
