a relative of mine had a blood clot in her leg so she is on blood thinners and when they drew blood it clotted in the tube what does this mean

There are a lot of things that can cause the blood to clot:
chronic liver disease
vitamin K deficiency
chronic kidney failure
certain drugs (tamoxifen)
leukemia
