Q: Bacterial or yeast infection ?

I've had an infection for quite a while... about a year. I think it is a bacterial or yeast infection and I really put off dealing with it because it really wasn't annoying at first, but now it has become not only annoying but painful, and I'm really embarrassed about it. I'm only 16.

If the infection goes away, does this mean my chances that I have gotten a disease such as Pelvic Inflammatory Disease are gone as well?
