Anyone Else Get Joint Pain When Laying Down?

I've had TMJ for 2 years. When I lay down, whether to read or sleep, after I get up I have horrible pain in my jaw joints and my jaw seems to be alot further back then normal. It happens when I lay down on my back and on my side. My guess is that my jaw joints are hitting nerves in the back.

I've gotten a $3000 orthogonal made that is bonded to my lower teeth and really helps, especially when I eat. I used to have pain all day, but now it is most discomfort. Now I only have pain when I wake because I've been laying down for so long, or when I do physical activity. This laying down issue seems to be the next hurdle I need to get over.

Anyone have any experience with this? My TMJ dentist doesn't know how to help with this issue, what type of doctor would I need to see for this?
