I really feel for you. I've had my share of back pain since 1982, when I had my first operation. I had a L4-5 spinal fusion w/full instrumentation and bone graft last June, and it helped me IMMENSELY!! As for the meds, I've been taking some form of oxycodone since 1987 when I had my second surgery, and was in chronic pain since then. I was initially taking percocet, then dr. didn't want me to take all that tylenol, so he switched me to Oxycontin. At one point last summer, around the time of my surgery, I was taking about 250-300mg of oxycodone daily. 2 40mg long acting, and about 6-7 30mg immed. release. I had had the same dr. since 1980, and he was wonderful. Understanding, compassionate, and trusting. But unfortunatley, he retired, and now I have a dr. who seems very suspect of my pain med usage. I just hate the attitude she has about them. I take appox 6 15 mg. per day, and I am able to work and raise a family now, but she told me the last time that "people sell these on the black market", and then she asked if I would take a urine test right there so she could prove that I am taking them!!!! I'm almost 50, a professional woman, and I couldn't even imagine selling my meds!! Now every time I have to go see her, she really harps on the issue. I read another chronic pain site on the internet, SpineHealth Forum, and I've seen many people post about the fact that their drs give them a really hard time about the pain meds, too. I think that really stinks, because the last thing you need when you're dealing with chronic back pain, is to worry about what if your dr. cuts you off for meds, and you might not be able to function??
Anyway, I just wanted to put my 2Cts in as far as your situation, with no health care. I would NOT go back to that pain clinic, it sounds like they are really in the business just to make a ton of money off people's suffering, and if you have a relationship w/your family dr., explain your situation to him and tell him that you will need to go on the most inexpensive meds while you are forced to pay cash for your meds, and see if he will agree to help you until you get a job w/coverage for possible surgery. I would highly recommend going to the closest university-type teaching hospital in your area, and see if you can find a highly skilled specialist-orthopeadic surgeon, that would take a look at your MRI's and give his opinion as to whether surgery would help you. There are many programs to cover people w/no insurance, and you should start out by checking your local hospital to see if they have a program for reduced rates for people w/no money. It is going to be tough for you to get a job if people think you have a bad back. I hate to say it, but I would keep that to myself, and try to get a job w/healthcare benefits.
I was so very surprised to find out that the techniques they use today for spinal fusion REALLY have a great outcome most of the time, especially for one-level fusions, as that's what it sounds to me like you would need. In the meantime, if you ask your dr. to just take care of rx'ing your oxy in a dose that works for you, I would go for that option rather than spend all that money for the pain clinic. I don't know how bad your pain is, but would you even be able to work on that much medication? It is up to you, but if I had the kind of pain that you have, I would be getting myself to a surgeon to see if you are a candidate, and to see if you can work out some kind of agreement on the cost. Maybe there'a program that would cover most of the cost of it because you are unemployed. In any case, I would forget about the pain clinic, and forget about the suboxone at this point, and get something strong to help you until you can have the surgery. I have heard that Suboxone does help w/pain, but if you end up having surgery soon, that will create a problem for them if you are taking it. Also, when drs hear you're taking THAT med, they will assume you are an addict, and that you had to take it to go off pain meds, and you will have a hard time getting any dr. to rx an opiate. Your family dr. sounds like the best bet if he knows & trusts you, and is willing to rx until you can get your back fixed, then you are very lucky. But I would get as many referrals to a great surgeon to give you their opinion, as soon as possible. That surgery changed my life. I was soooo scared to do it, and now I wish I did it years ago!! Life is short and you don't want to waste too much time unemployed on pain meds, cause it will surely be hard for you to get out and get yourself a job after being off for that long. If an employer sees that you've been out of work so long, I'm sure they'll wonder why? So go and be pro-active!! GEt your pain meds from your dr. until you get your back fixed, then at THAT point you may want to consider suboxone to help you get off the oxycodone.
It really helped me to read other people's stories, and suggestions. You should check out that other site I mentioned above, too, and learn as much as possible about your own situation. I hope that everything works out for you, pain is a very lonely place-I know!! Good luck to you!!
