Q: acute pleurisy chest pain

What started out on 12/7/08 as a cold, turned into a sinus infection & pink eye. I had an allergic reaction to the azithromycin antibiotic.
On 12/27/08 I had to go to Urgent Care because the chest pain was so bad. I was informed that I had acute pleurisy and acute bronchitis, along with a case of GERD. Ok, fine. Went on Levaquin and OTC Maalox and that's it, it is supposed to be gone, right?
I've been telling the doctors at Urgent Care and the ER that I have the following symptoms and I was told the end of January that these should clear up.
What do you think?

1. I get out of breath from talking more than a minute or two
2. I huff and puff from putting on socks and shoes or bending
3. cannot sleep on my back, feels like a ton of bricks on my chest
4. dizzy 24/7 for the last 10 weeks
5. extremely fatigued, no energy to do normal every day things like pick up a gallon jug of water, do a load of clothes, whatever

I have had pleurisy about 7-8 times now as an adult. Your symptoms - #1,2,3 and #5 are EXACTLY all symptoms of the pleurisy. I also get ear pain when I breathe as well, which is related.
