Q: ACDF c5-c7 new herniation

I underwent an acdf it will be 2 yrs. Febuary. I have had many complications since the first one directly following the fusion I had dysphasia & dystonia with repeated choking episodes & laryngospasms. This went on for a yr. finally had an EMG pharynx, & esophageal manmoentry, as well as a videofluroscopy. The dx was R sided (that's where the incision for the fusion was done) paresis. This dx caused me to has significant wt loss & dehydration. I ended up with a PEG tube in place. Now workman's comp independent medical advisor says this does not happen from the surgery & all feedings suction machine at home will be denied. There is also on my latest MRI a new disc herniation adjacent to C6-7 its C7 T1 with C8 nerve root involvement causing significant pain. There is also a chance of the screw at C6 backing out due to not enough bone to hold it in place. If the screw is backing out what is the treatment for this? Will I need to have another surgery? I did have prior to the ACDF a foraminotomy posterior on the L side due to severe radiculpoathy. I was until 3 yrs ago an ER trauma nurse with over 23 yrs experiance, I am currently 44 with 4 teenagers I used to have all the answers now it seems I don't know anything.
