a bad rash extremely itchy and scaly, it's also very red

I have a rash on both of my breasts on the underside same exact area on both. I've tried using medicated creams, fungal sprays, powders and other fungal creams and nothing seems to be clearing it up. It's extremely itchy and scaly it's also very red and the skin is raised up where the rash is on my left breast. It also started bleeding today when I scratched it, it just looks like a patch of really dry skin
